Active Stanford University employees may access their IRS Form 1095 securely by using the instructions below:

Log on to the My Benefits portal on (Cardinal at Work).

From the employee homepage, scroll down to "Your Benefit Providers" and select the Conduent tile.

You will be taken to a page that displays all of your IRS Forms 1095. You may print or download your available forms at any time from this site. Alternatively, you may request a paper statement be mailed to you by selecting "Mail Reprint" from the menu.

Former employees and anyone else unable to access My Benefits portal can Click here to directly log into the HealthAct portal.

If you need to update your mailing address, provide an additional email address, or have questions regarding your Form 1095, please contact the University HR Service Team by calling (650) 736-2985 or (877) 905-2985, Monday through Friday, 8:00 am to 5:00 pm PT (excluding holidays).
